3. Tongue and pulse

• Tongue: Pale tongue body, white coating.
• Pulse: Thin and weak, or deep, slow, and forceless.

4. Common modifications

• For severe pain with predominant cold pathogen: add Zhi Chuan Wu (prepared aconite root) and Zhi Cao Wu (prepared wild aconite root) (both decocted first for a prolonged time) and Bai Hua She (white-banded snake) to strengthen the ability to dispel wind, disperse cold, and relieve pain.
• For predominant damp pathogen with heaviness of the limbs: add Fang Ji (stephania root) and Cang Zhu (atractylodes rhizome) to dispel dampness.
• If root deficiency is not severe, the tonifying herbs such as Di Huang (rehmannia root) and Ren Shen (ginseng) may be reduced as appropriate.
• Modern applications: Widely used for chronic arthritis, rheumatoid arthritis, sciatica, lumbar muscle strain, and bone hyperplasia (osteophyte formation), where these present with long-standing wind-cold-damp painful obstruction, combined Liver-Kidney deficiency, and insufficiency of qi and blood.

5. Cautions and clinical notes

• Contraindications:
  • Contraindicated in painful obstruction of the damp-heat excess type, presenting with red, swollen, hot, and painful joints, dry mouth, and yellow greasy tongue coating.
  • Use with caution in pregnant women.

6. Granule conversion and dose

• Calculation of total raw herb weight in the original formula:
  • Du Huo (pubescent angelica root) 9 g + Sang Ji Sheng (mulberry mistletoe) 18 g + Du Zhong (eucommia bark) 9 g + Niu Xi (achyranthes root) 9 g + Xi Xin (asarum herb) 3 g + Qin Jiao (large gentian root) 9 g + Fu Ling (poria) 9 g + Rou Gui Xin (cinnamon bark, inner layer) 6 g + Fang Feng (siler root) 9 g + Ren Shen (ginseng) 9 g + Gan Cao (licorice root) 6 g + Dang Gui (Chinese angelica root) 9 g + Bai Shao (white peony root) 9 g + Gan Di Huang (dried rehmannia root) 15 g + Chuan Xiong (Sichuan lovage rhizome) 6 g = 135 g.
  • (Note: In clinical practice, Dang Shen (codonopsis root) is often substituted for Ren Shen, and the dose may be adjusted accordingly.)
• Conversion to 5:1 granules:
  • 135 g raw herbs ÷ 5 = 27 g granules.
• Recommended dose:
  • One dose daily, taken in 2 divided doses (morning and evening) dissolved in warm water, about 13.5 g per dose.
  • Or as directed by a physician. This formula is used for chronic conditions and must be taken over a prolonged period before effects are seen. The physician will make fine adjustments according to the relative strength of the pathogen and the patient's underlying deficiency; in that case, the total raw herb weight should be recalculated based on the adjusted formula.
