四逆汤

Sheng Fu Zi (raw aconite root) 15 g + Gan Jiang (dried ginger rhizome) 9 g + Zhi Gan Cao (honey-fried licorice root) 6 g = 30 g.

Conversion to 5:1 granules:

30 g raw herb ÷ 5 = 6.0 g granules.

Granule dosage: This is an emergency-use formula; strict adherence to a practitioner's instructions is essential. General reference: taken in 2-3 doses daily, dissolved in warm water, about 2.0-3.0 g per dose.

5. Cautions and clinical notes

• Contraindications:
  1. This formula is a potent formula for restoring yang and rescuing from collapse, suitable only for cold reversal (han jue) due to yang decline with abundant yin. It is absolutely contraindicated in cases of true heat with false cold (heat reversal, re jue).
  2. Contraindicated in pregnancy.
• Safety warnings:
  1. Fu Zi (aconite root) in this formula is highly toxic, especially raw aconite, and must be used under a practitioner's guidance. In preparing ProCare Herbs granules, only processed aconite that has been properly prepared according to pharmacopoeia safety standards must be used, with strict dosage control to ensure safety of use.
  2. If signs of toxicity appear, such as numbness of the lips, tip of the tongue, or fingertips, nausea, or palpitations, discontinue use immediately and seek medical attention.
• Modern applications: Mainly used for shock of various causes, heart failure, myocardial infarction, and collapse due to excessive vomiting and diarrhea from acute gastroenteritis, in cases presenting the pattern of yang decline with abundant yin.
• Duration of use: This formula is for emergency use; discontinue once the condition resolves. Once yang qi has been restored, the formula should be adjusted, and it should not be taken for a prolonged period.
