清咽利膈汤

• Standard adult dose:

o Total daily dose is 20 g of granules.

o Taken in two doses: 10 g per dose, twice daily. Take slowly with warm water after meals.

• Frequent dosing in acute cases: During acute flare-ups, frequency may be increased to 3 times daily as appropriate.
• Stop once the condition resolves: This formula is a purgative, attacking formula; once effect is achieved (bowel movement restored, heat resolved, pain reduced), the dose should be reduced or the formula discontinued.

5. Cautions and clinical notes

• [Contraindications]:
  1. This formula is strongly cold and purgative in nature; it is absolutely contraindicated in those with Spleen-Stomach deficiency cold and in pregnant women.
  2. Contraindicated in those with yin deficiency and effulgent fire (presenting with a red tongue with scanty coating and a thin, rapid pulse).
  3. Use with caution in the weak and elderly.
• [Key point]: Use of this formula requires identifying the pathomechanism of "excess, heat, congestion, and blockage"; it should only be used when there is definite interior excess heat-bind presentation such as red, swollen throat, fever, and constipation.
• Modern applications: Mainly used for acute tonsillitis, acute pharyngitis, laryngitis, stomatitis, and mumps where the pattern is Lung-Stomach heat toxin congestion.
• Duration of use: This formula is designed to urgently purge to preserve yin and to abruptly break the course of the disease; it should be stopped once effect is achieved. Generally taken for 1-3 days; it should not be taken long term.
