o Binding of the three: Phlegm, qi stagnation, and blood stasis bind together along the course of the Liver channel in the anterior neck, giving rise to goiter and nodules (ying liu).

- Exuberant Liver fire (bitter taste in the mouth, irritability): Add Xia Ku Cao (prunella spike), Zhi Zi (gardenia fruit), and Dan Pi (moutan bark) to clear the Liver and drain fire.
- Yin deficiency with effulgent fire (dry mouth, palpitations): Add Mai Dong (ophiopogon root), Sha Shen (glehnia/adenophora root), and Sheng Di (raw rehmannia root) to nourish yin and lower fire.
4. Duration of use and monitoring:
o Goiter and nodules (ying liu) are chronic conditions that are difficult to resolve in the short term. Duration of use should be determined under a practitioner's guidance, and is usually measured in months.
o During treatment, thyroid ultrasound should be reviewed periodically (e.g., monthly) to observe changes in the size and nature of the mass, so that the treatment plan can be adjusted promptly.
o 气滞:情志不遂,肝气郁结,是发病的始动因素。
