4. Granule conversion and dose

Calculation of the total raw herb weight in the original formula:

Sheng Di Huang (rehmannia root) 12 g + Mai Dong (ophiopogon tuber) 9 g + Xuan Shen (scrophularia root) 9 g + Chao Bai Shao (stir-fried white peony root) 5 g + Mu Dan Pi (moutan bark) 5 g + Chuan Bei Mu (fritillaria bulb) 4 g + Bo He (mint) 3 g + Sheng Gan Cao (raw licorice root) 3 g = 50 g.

Conversion to 5:1 granules:

50 g raw herb ÷ 5 = 10.0 g of granules.

Granule dose: Dissolve in warm water and take in 2 doses, morning and evening, about 5.0 g per dose. (The original formula was used for acute conditions; the frequency of dosing may be increased as appropriate.)

5. Cautions and clinical notes

[Contraindications]:

1. Use with caution or avoid in those with Spleen deficiency and loose stools. The herbs in this formula are mostly sweet, cold, and rich in nature, and may readily impede Spleen-stomach movement and transformation.
2. In early-stage diphtheria accompanied by an exterior pattern (wind-cold or wind-heat), exterior-releasing herbs should be combined in; this formula should not be used alone.
3. Not suitable for throat swelling and pain caused by exuberant damp-heat.

[Safety warnings]:

1. This formula is a therapeutic preparation and should be discontinued once the condition resolves.
2. Modern diphtheria is an acute infectious disease; if suspected, medical care should be sought immediately and comprehensive treatment measures taken, with this formula used only as an adjunct.

Modern application: Mainly used for acute tonsillitis, acute pharyngitis, laryngitis, and dryness of the mouth and throat after radiotherapy for nasopharyngeal carcinoma, in patterns identified as lung-kidney yin deficiency with deficiency fire flaring upward. Its application has extended well beyond diphtheria, becoming a classic formula for throat disorders of the yin-deficiency lung-dryness type.

Duration of use: For acute throat conditions, typically taken for 3-7 days. Discontinue once symptoms have resolved. For regulation of chronic conditions, may be taken for 2-4 weeks as directed.
