3. Tongue and pulse

• Tongue: Red tongue body, scanty coating.
• Pulse: Wiry and fine, or fine and rapid.

4. Common modifications

• As a patent formula, the composition of this product is fixed. When physicians use it clinically as a base formula for modification:
  • For severe yin deficiency with the five-center heat (heat in the palms, soles, and chest): add Zhi Mu (anemarrhena rhizome) and Huang Bai (phellodendron bark) — following the idea of Zhi Bai Di Huang Wan — to nourish yin and reduce fire.
  • For pronounced Liver fire with bitter taste in the mouth and rib-side pain: add Zhi Zi (gardenia fruit) and Long Dan Cao (gentian root) to clear and drain Liver fire.
  • For Heart blood insufficiency with insomnia and palpitations: add Suan Zao Ren (sour jujube seed) and Yuan Zhi (polygala root) to nourish the Heart and calm the spirit.

5. Cautions and clinical notes

• Contraindications:
  • This product is designed for yin deficiency with effulgent fire and Liver yang rising in excess; it is therefore contraindicated for sudden-onset tinnitus or deafness of excess type, such as that caused by external wind-heat invasion or Liver fire flaring upward.
  • Use with caution in those with Spleen-Stomach deficiency cold, poor appetite, and loose stools.
  • Avoid spicy and greasy foods while taking this medicine.

6. Granule conversion and dose

• Calculation of the total raw herb weight of the original formula:
  • Shu Di Huang (prepared rehmannia root) 24 g + Shan Zhu Yu (cornus fruit) 12 g + Shan Yao (chinese yam) 12 g + Ci Shi (magnetite) 20 g + Wu Wei Zi (schisandra fruit) 6 g + Fu Ling (poria) 9 g + Ze Xie (alisma rhizome) 9 g + Dan Pi (moutan bark) 9 g + Zhu Ye Chai Hu (bamboo-leaf bupleurum root) 6 g = 107 g.
• Conversion to 5:1 granules:
  • 107 g raw herbs ÷ 5 = 21.4 g granules.
• Recommended dose:
  • One dose daily, taken in two portions, morning and evening, dissolved in warm water, about 10 g per dose.
  • Or as directed by a physician. Tinnitus and deafness are chronic conditions, and sustained use over a period of time is needed before effects are seen. Please strictly follow the physician's prescription.
