4. Granule conversion and dose

Calculation of total raw herb weight in the original formula:

Jin Yin Hua (honeysuckle flower) 90 g + Xuan Shen (scrophularia root) 90 g + Dang Gui (Chinese angelica root) 30 g + Gan Cao (licorice root) 15 g = 225 g.

Converted to 5:1 granules:

225 g raw herb ÷ 5 = 45 g granules.

Granule dose: Dissolve in warm water and take in 2 doses, morning and evening, about 22.5 g per dose. (The dosage in the original formula is extremely large; this is a special dosage for severe, acute conditions and must be used strictly under a practitioner's guidance.)

5. Cautions and clinical notes

• Contraindications:
  1. This formula is suitable for yang-type, excess patterns of blazing heat toxin. It is absolutely contraindicated for tuo ju caused by cold-congealing yin conditions (presenting with a cold, painful affected limb and pale skin).
  2. Use with caution in those with Spleen-Stomach deficiency cold.
• Key point: The key to applying this formula is confirming a pattern of blazing heat toxin. Its typical presentation is redness, swelling, heat and pain at the affected site, with ulceration and putrid odor. This formula is cold and cooling in nature; discontinue once the disorder resolves, and it should not be taken long-term.
• Modern applications: Mainly used for thromboangiitis obliterans, arteriosclerotic occlusive disease, diabetic foot, erysipelas, cellulitis, and other conditions where the pattern is blazing heat toxin.
• Duration of use: This formula addresses the acute manifestation as a matter of urgency, used to control severe infection and necrosis in the acute stage. Once symptoms are relieved, the dose should be reduced or the formula changed immediately; it should not be taken long-term as a maintenance formula.
