4. Granule conversion and dose

• Calculation of the total raw herb weight of the original formula (excluding honey):
  • Huo Ma Ren (hemp seed) 20 g + Da Huang (rhubarb rhizome) 12 g + Zhi Shi (unripe bitter orange) 10 g + Hou Po (magnolia bark) 10 g + Bai Shao (white peony root) 10 g + Xing Ren (apricot seed) 10 g = 72 g.
• Conversion to 5:1 granules:
  • 72 g raw herb ÷ 5 = 14.4 g granules.
• Recommended dose and method of administration:
  • One dose daily, taken in two portions dissolved in warm water.
  • It is recommended to take on an empty stomach or before meals, so that the action of the medicine reaches the intestines directly.
  • If it is desired to follow the intent of the classical formula, the granules may be dissolved and taken with an appropriate amount of honey water, to strengthen the moistening and downward-draining action.
  • Each dose is approximately 7 g.

5. Cautions and clinical notes

1. [Contraindications]: Although this formula is a moistening and downward-draining formula, it still contains substances of a purgative, breaking nature; therefore, in the elderly and debilitated, in those with depleted fluids and blood, and in constipation of pregnancy, it should not be used for a prolonged period. It is contraindicated in those with pure deficiency and no excess.
2. Dietary cautions: During treatment, plenty of water may be drunk, and fruits and vegetables rich in fiber may be eaten liberally, to assist bowel movement.
3. Modern applications: This formula is widely used for habitual constipation, constipation in the elderly and postpartum women, and constipation following hemorrhoid surgery, in cases belonging to the pattern of intestinal and Stomach dryness-heat.
4. Duration of use: This formula treats the symptom rather than the root; once the stool passes freely, the medicine should be discontinued, or the formula should be adjusted to address the underlying cause. It should not be relied upon for a prolonged period, to avoid the development of tolerance.
